Suicide Risk Questionnaire for Adolescents
02.07.2026

Suicidal thoughts and tendencies during adolescence can often go unnoticed by others. This period in a person's life, known as the transitional age, is a crisis period from the point of view of psychology. During this time, adolescents are particularly prone to internal contradictions, excessive emotionality, a tendency to seclusion and a sense of acute dependence on the opinions of others.

Who should take this test and why?

This questionnaire was modified by T. Razuvaeva and is designed to identify the level of risk of suicidal tendencies in adolescents aged 12 to 18 years. The main purpose of the test is to assess the depth of suicidal intentions and to prevent possible rash actions leading to suicide. It is crucial to distinguish between what motivates an adolescent: the desire for demonstrative attention or the presence of deep suicidal thoughts.

Who uses this test?

This questionnaire is actively used by psychologists in educational institutions and social psychologists as a quick way to assess suicidal risk among adolescents.

Instructions for taking the questionnaire

While taking the survey you will be presented with 29 statements concerning your inner world, fears, views on the future and other aspects. If you agree with the proposed statement, select the "yes" option. If the statement does not match your perception of the world, select "no". The honesty of your answers will provide the most accurate understanding of your condition and suicidal tendencies.

Survey Results

The questionnaire includes an analysis of nine key factors associated with suicide risk. Based on your responses, the degree to which each of these factors is manifested in you personally will be assessed.

Interpretation of results

Demonstrativeness

The desire to draw attention to one's problems in order to gain support, sympathy and empathy. This may look like emotional blackmail and exaggeration of difficulties, but is actually a sign of a request for help. It is especially dangerous when this behavior is combined with obsessive attention to the problem, indicating emotional inflexibility.

Affectivity

The dominance of emotions over logic and reason in evaluating situations. Characterized by emotional impulsivity.

Uniqueness

A sense of the exclusivity of one's life, one's self, and the circumstances in which one finds oneself. Often involves finding an unusual solution to a problem, such as suicide.

Failure

A sense of one's own irrelevance and worthlessness in the world, lack of faith in oneself as a person.

Social pessimism

Perception of the world as unfriendly and rejecting, denial of the importance of society.

Breakdown of cultural barriers

Legalization of suicidal behavior through cultural examples from movies and literature, or imitation of famous individuals who have committed suicide. This leads to the maximum manifestation of the idealization of the role of the owner of one's destiny, the mixing of the concepts of "life" and "death", and the cult of death.

Maximalism

A tendency to exaggerate failures, transferring them from one area of life to others, with the conviction that further failures are inevitable and that there are no ways to correct them.

Temporal perspective

Difficulty planning for the future due to a focus on the current moment and fear of future failure.

Anti-suicidal factor

The presence of this factor can significantly reduce the risk of suicide, even if other factors are pronounced. It is based on the awareness of responsibility to loved ones, fear of causing them mental pain, as well as the fear of physical pain and unaesthetic act of suicide, as well as thinking of it as a sin.
